Biotin and Lab Test Interference

The direction of the error depends on the assay format:

The FDA issued a safety communication in November 2017, updated in 2019, warning that biotin can significantly interfere with certain lab tests. The alert was prompted in part by at least one reported death linked to a falsely low troponin result in a patient taking high-dose biotin – the test missed an active heart attack [2].

The most clinically significant interferences include:

Thyroid function tests

This is the most commonly reported problem. High-dose biotin can produce a pattern that mimics Graves’ disease on paper:

  • Falsely elevated free T4 and free T3
  • Falsely low TSH

Multiple case series have documented patients being investigated for – or even treated for – hyperthyroidism that did not exist, simply because they were taking biotin supplements [3][4].

Cardiac biomarkers

  • Troponin – falsely low results could mask an active myocardial infarction (heart attack)
  • BNP/NT-proBNP – used to diagnose heart failure; false results could delay treatment

Hormone panels

  • Testosterone, estradiol, cortisol, DHEA-S, vitamin D metabolites, and other hormones measured by biotin-streptavidin assays can be affected

  • PSA (prostate-specific antigen)
  • Ferritin
  • Certain tumor markers
  • Parathyroid hormone (PTH)

At what dose does interference occur?

The threshold is lower than many people assume. The American Thyroid Association notes that interference has been reported at biotin blood concentrations as low as 10 ng/mL, which can result from daily doses of just 5 mg (5,000 mcg) – a common over-the-counter supplement dose [5].

Many popular biotin supplements provide 5,000-10,000 mcg per serving. Some marketed for hair growth contain even more.

How to manage the risk

Before blood work

  • Stop biotin supplementation at least 48-72 hours before any lab tests
  • For high doses (5,000 mcg or more), some experts recommend a 5-7 day washout
  • Biotin is water-soluble and clears relatively quickly, but higher doses take longer

Patients with multiple sclerosis
